Due to critical service need of the communities we serve, we are offering a $4,000 sign-on bonus for Nurses. We are eager to continue to provide quality and compassionate care in our service areas and need you to make this happen!
The Medication Adherence Nurse assists clients that have missed their scheduled appointment with a Psychiatrist or APN and needs to be rescheduled. This individual also assists clients with obtaining a medication refill that is sufficient in maintaining on the same medications previously prescribed to them until such date that appointment has been rescheduled for client.
ESSENTIAL FUNCTIONS
Communicates with clients requesting a medication refill to ensure there are no medication gaps with critical medications and appointment scheduled for needed refills.
Works closely with the Centralized Scheduling department to reschedule a client in the next available opening of the current Psychiatrist
If medication refill is required before new appointment is scheduled the coordinator researches the client medical record and obtains a listing of present medications and dosages
Communicates the medication refill needs with Psychiatrist or APN and helps facilitate obtaining an urgent refill for those medications that is sufficient until next appointment date
In the event the clients current Psychiatrist or APN is not available, the coordinator will work with another Medication Refill physician
Collaborates with insurance companies, pharmacies, and clients for medications that need prior authorizations
Oversees the order console, verifying prescriptions make it through the system to the pharmacy
Assist with medication adherence CCBHO outcomes measure. Provide education to clients on medication adherence.

EDUCATION
High School/GED required
Practical Nursing certificate required
WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS
At least two (2) years experience working in a mental health setting preferred
LICENSURE/CERTIFICATION
LPN or GPN that must pass the MO NCLEX exam within 90 days of graduation required
