Medical Assistant Health Coach REMOTE

What Youll Do:
Make an Impact Daily : 60+ outbound calls a day to engage and activate patients.
Create Meaningful Connections : 19+ successful patient interactions per day.
Coach & Motivate : Use motivational interviewing to drive positive health changes.
Close Gaps, Change Outcomes : Help patients achieve SMART health goals and close HEDIS/care gaps.
Identify & Provide Support : Assess Social Determinants of Health (SDOH) challenges and connect patients to community resources and tools that improve their well-being.
Ensure High-Quality Care : Maintain a 90%+ QA score through effective communication.
Celebrate Success : Capture and share real patient stories that inspire others.
Who You Are:
3+ years of experience as a Medical Assistant (MA)
Passionate about patient engagement & health coaching
Strong in chronic disease management & care coordination
Skilled at EHR/EMR navigation & documentation
Empathetic, driven, and a problem-solver
