PRIMARY FUNCTION

The Housing Stability Manager (HSM) has the responsibility to ensure the intermediate and long term residential stability of all housing residents. To promote ASDs mission and goals, to respect the confidentiality of all who come to us for information or assistance, and to personally maintain the dignity and integrity of one who is placed in a position of public trust. The Housing Stability Manager continuously works on helping to improve the organization and on communicating openly.

The focus of this position will be to ensure efficient delivery of quality case management services to clients through assessment, planning, service procurement, delivery, coordination, and monitoring, ensuring all services are offered in an inclusive, respectful and delivered in a professional manner. The HSM reports directly to the Chief Operating Officer and is required to attend supervisory and case conferences and other meetings as directed. He/She is a member of the Quality Management team. The HSM is to ensure that all housing related direct care staff client data be accurate and up to date. Because the position manages confidential information for clients, the HSM must demonstrate the highest degree of professionalism. This position is full time and requires travel to and from housing facilities.

DUTIES AND RESPONSIBILITIES

• Establish annual occupancy and tenure goals for all residents.
• Work to access resources within the community to enhance the education and skill set of all direct care housing providers within ASD.
• Continue to promote and work to enhance the community awareness of the value of ASD housing services and opportunities with statistical summaries quarterly.
• Coordinate client care with medical, mental health, substance abuse and other providers.
• Update and train staff on the day to day progression of client success and goals met in accordance with programmatic and funding requirements.
• Establish and monitor the care plan and growth path on all residential clients.
• Work with outreach and vocational to maintain a clear ongoing path of success for all residents.
• Formally produces linked summary data that encompasses all provided support, assessment, and case management services to individuals and families.
• Gathers information on residents through personal interviews and interviews with families, physicians and other service providers.
• Ensures that each housing participant through Case Management staff is actively trained on Housing Readiness, Eviction Prevention, Ready to Rent and are in pursuit of permanent affordable housing options that do not require Case Management, specifically Dallas Housing Authority.
• Train staff to refer residents to appropriate community agencies for further assistance and provides discharge planning to ensure post-resident care.
• Produce quarterly reports on Housing Stability.
• Works with HUD HMIS data APR to ensure performance measures are met at least quarterly.
• Provides outreach and information to the community regarding housing graduates and success strategies proven by program participants.
• Work with interns as provided to support housing stability.
• Provide assistance as requested by the Vice President/COO.
• Ensures the CM team participate in intake planning including, but not limited to, intake calls, off-site interviews, home visits and discharge planning.
• Report care plan measurable statistics for residents at least quarterly.
• Report on Case Manager Efficiency of client outcomes per case load. Recommend improvements as needed on client outcomes.
• Work closely with all departments to ensure client success.
• Participate actively in any fund raisers provided by ASD.
• Respond to calls, and/or, emergencies at any time.
• Must work physically on site.

QUALIFICATIONS

• BA/BS required in social work, human services, or related field, Master Degree Preferred in MSW, LCSW or three years of experience.
• Experience in psychology, social services and particularly in the field of HIV/AIDS care is highly desired. Preference will be given to those applicants who have experience providing case management to populations with issues including mental illness, substance abuse, chronic health and/or homelessness.
• Public Speaking and familiarity with database, internet, Word, Excel, and Outlook.
• Access to reliable transportation required.
• Fluency in a second language is a plus but not required.

WORK HOURS:
Regular office hours are 8:30 a.m. to 5:00 p.m. Monday through Friday.

SALARY RANGE:

$55,000.00 $65,000.00 per year
