Job Schedule: Full-time (40 hours/week)

Salary Range: $32.00 - $48.00

About the Role

Become a vital part of our expanding Kidney Transplant team! As a Transplant Coordinator , you will work closely with our post-transplant patients, providing essential education and coordinating their care.

Our clinic operates Monday through Friday from 8 AM to 5 PM, with on-call expectations shared evenly among coordinators for days, nights, weekends, and holidays. We are open to hiring for 32 to 40 hours a week.

Key Responsibilities

  • Coordinate efforts between the transplant center and major patient referral sources (e.g., Dialysis Units and Nephrology Clinics).
  • Assume a case manager role to ensure timely and efficient transplant education and updates for potential recipients and donors.
  • Manage a designated caseload of patients throughout the pre/post-recipient and pre/post-donor transplant processes.
  • Address roadblocks in the transplant workup process and provide educational programming for patients, families, and staff.
  • Ensure compliance with policies, procedures, and regulatory requirements for the transplant program.
  • Provide outreach services to regional dialysis and nephrology programs.

Qualifications

  • Graduate from an accredited nursing program (AACN, ACEN, NLN CNEA).
  • Bachelor’s Degree in nursing required, or in progress with an estimated completion date within five years.
  • Current unencumbered RN license with the State Board of Nursing; multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Must obtain a Minnesota RN license within six months of starting the position.
  • Maintain required department-specific competencies and certifications.
